Standardised resolution label for loggraph use

Fix from Steve Ernst for setting bin width when using WIDTH FBINR option.

Changed RESOLUTION keyword function: now determines which reflections are
excluded from final statistics. Also minor changes to format of log output.

1. RESOLUTION keyword no longer able to reject reflections from output; all
   reflections written to output mtz.
2. Scale and temperature factors now calculated consistently in final
   (output) cycle.
3. Modified/added various messages to log file output.
4. Increased parameter MaxResBin (A.Leslie)
5. Fix for NINT (should be INT) in binning reflections by resolution (A.Leslie)
6. Fix for assigning maximum resolution (A.Leslie)

EJD : check for SMIN and DMIN = 0.0

EJD: After SHELX you can have a file where EVERY FREE reflection is
  missing..  Here is a fix.

EJD   OUTPUT FOFC now copies all assigned columns to HKLOUT;
  ie SIGFC and FREE are not lost

EJD: Using PROCESS FBC the width was multilpied by the scale every time a
reflection was read; after 28751 reflections we got an overflow..

Avoid undefined var. error with absent sigmas -- should be checked;
Add END keyword

Kim made corrections (don't know what for) and reformatted code, changed
variable names.  Re-reformatted it as already beyond recall.
