Add/change history info writing for a bunch of programs.  Mostly call the
new LWHSTL.  Several previously called incorrectly LWHIST with scalar
instead of character array.

I have made 2 main changes: NaNs if present in the MTZ file are handled
properly (though some changes are also required in MTZLIB so that LRREFL
and LWREFL pass the NaNs without attempting to do arithmetic on them); and
the new mtztona4 can optionally (by setting CCP4_RLENA4) produce a more
compact run-length encoded format, which naturally the new na4tomtz can
unscramble.

I've made the default the original format, to give time for people to
upgrade their na4tomtz, and mtztona4 issues a warning message (ie ensure
that the intended recipient of the file has the right version of na4tomtz),
though I don't suppose anyone will bother to read it.  If the old na4tomtz
is used on the RLE format, it just ignores all the data (I changed the
end-of-header marker).  Of course the new na4tomtz automatically detects
the old NA4 or the new RLE, it doesn't need CCP4_RLENA4 to be set.
